We are doing something different—something that changes the way we care for seniors and measure success. Imagine a senior leaving the hospital. What happens next? Many studies focus on numbers like how many seniors take their medications or how long they stay in one place, but we look deeper. We focus on what really matters: Are they thriving? Are they happy? Are they in the right environment to stay healthy and out of the hospital?

Our study measures how well seniors adjust to their new care environments and how those environments help them avoid going back to the hospital. We track hospital readmissions over 30, 60, and 90 days. Why? Because every time a senior ends up back in the hospital, it’s stressful for them, their families, and the healthcare system. We also look at how many unnecessary trips to the emergency room are avoided because seniors are in settings that provide personalized attention and quick action when issues arise.

But we don’t stop at medical outcomes. We measure how well seniors are living day to day. Are they making friends? Are they enjoying meals that remind them of home? Do they feel like they belong? We pay attention to things like language, culture, and diet to ensure they feel comfortable in their new environment. When seniors feel at home, they heal faster, stay healthier, and are less likely to need emergency care.

Family satisfaction is another big piece of what we measure. We know how hard it is for families to trust that their loved ones are being well cared for. That’s why we ask families how confident they feel in the care their loved one is getting. When families are at ease, they can focus on being present and supportive instead of worrying.

What makes our study stand out is that we take a holistic approach. We’re not just measuring whether seniors take their medicine or how long they stay out of the hospital. We’re looking at their overall experience—physical health, emotional wellbeing, and quality of life. This isn’t something other studies typically do. Most focus on the clinical side, but we combine medical outcomes with human outcomes. That’s what makes us a unicorn.

Our approach is about connection. We bring together hospitals, senior living communities, and other care providers to form a seamless network of support. This collaboration ensures that seniors are placed in environments that truly meet their needs. Hospitals and senior living operators care about these results because they directly impact their goals—reducing costs, improving satisfaction, and keeping seniors healthy.
